We are pleased to announce the launch of the 2024/25 Quality Improvement Plan (QIP). Ontario Health’s QIP program supports more than 1,000 organizations from the hospital, long-term care, and interprofessional primary care sectors to use a systematic and transparent quality improvement process to transform care.
Objectives
There are 4 million caregivers across Ontario, providing an estimated 75% of the care to family members, partners, friends, or neighbours. Caregiver well-being has an impact on patient health outcomes – well-supported caregivers are more likely to provide better care. Primary care providers can play a key role in preventing burnout and enhancing caregiver well-being by initiating early interventions and connecting caregivers to support.
Uncertainty and unpredictability have been constants throughout the duration of the pandemic as has the need to navigate transitions. In this webinar, participants will have the opportunity to gain insight into change processes and their impact on workplace mental health. Strategies for managing hybrid working models and combating stress will also be explored.

Concussions are a common complication of head trauma and a frequent reason for emergency department visits. In the pediatric population alone, there are nearly 50,000 diagnosed concussions in children ages 5-19 each year, commonly occurring from sports-related injuries. But a CT scan is often unnecessary for concussions and might even cause more harm than good.
Ontario Health will provide information and answer questions about FY2021-22 digital and virtual care provincial funding opportunities for Ontario Health Teams (OHTs) and In Development Team, including support for online appointment booking and virtual primary care projects. This funding is available to support health system recovery, accelerate health system transformation, and build OHT digital maturity.
