Supporting high-quality interprofessional primary care
AFHTO members provide comprehensive primary health care to over 3 million patients. That’s nearly a quarter of all Ontarians.
Who can be an AFHTO member?
An organization is eligible to become an AFHTO member if it:
- Provides comprehensive primary care
- Is interprofessional and team-based
- Has an administrative structure/agreement that coordinates the relationship among team members
- Operates in and is primarily funded by the Province of Ontario
AFHTO’s membership includes Family Health Teams, Nurse Practitioner-Led Clinics and other interprofessional primary care organizations.
Primary care teams show the whole is greater than the sum of its parts, such as:
- Wawa Family Health Team won the 2022 Bright Lights award for expanding their Medical Abortion Program to include a virtual component, serving dozens of urban women, in addition to local patients.
- Petawawa Centennial Family Health Team won the 2023 Bright Lights award for creating the Integrated Virtual Care (IVC) to address the shortage of family physicians in the Renfrew County area. This hybrid model of care increased access to comprehensive, team-based primary care while improving equity.
- City of Lakes Family Health Team won 2024 Bright Lights award for leveraging AI to reduce primary care providers’ administrative burden, addressing privacy and equity concerns while improving work-life balance.
