Objectives

Clinicians are caring for people with increased food and housing insecurity, isolation and loneliness, and declines in mental health and at the same time, are often left unprepared and unsupported in addressing social needs.

This fireside discussion will unpack the global conversation on social prescribing and how it can apply to practice in Ontario. Social Prescribing brings together the social and medical models of health, allowing us opportunities to care for people in a more careful and caring way by shifting the lens from "What's the matter with you?" to "What matters to you?" At last year's Family Medicine Forum, social prescribing was voted in the top two "Big Ideas" that would change the future of family practice.

Join us as we will explore how this global concept makes a big difference in diverse Ontario practice settings, including primary care teams. Along the way, we will share practical ways to identify and address social needs within the clinical setting through social prescribing.

Presenters

Natasha Beaudin

  • Natasha Beaudin has worked in the Community Health Centre sector for more than 20 years, most recently as a health promoter at Centretown Community Health Centre in Ottawa
  • She has worked in food security, smoking cessation, stress reduction, and physical activity, as well as leading the social prescribing project for her centre
  • She chaired the health promotion network of Eastern Ontario (HPNEO) for 5 years
  • She is currently the lead on social prescribing at the Alliance for Healthier Communities

Dr. Dominik Nowak

  • Dr. Dominik Nowak (DOM-in-ik NO-vahk) is a family physician at Women's College Hospital
  • In addition to his clinical practice, Dr. Nowak's mission is to build teamwork across the health system
  • As part of that mission, he is active in advancing social prescribing, most recently leading the creation of a first-of-its-kind resource for health professionals with the Alliance, the Centre for Effective Practice, the Canadian institute for Social Prescribing and St. Mike's

Margo Reilly

  • Margo Reilly is a seasoned healthcare leader in Windsor-Essex County, spanning diverse environments, including rural and urban settings. She is the Executive Director at the Windsor Family Health Team and serves on the Executive Committee at Windsor Regional Hospital.
  • Throughout her career, Margo has been passionate about creating innovative and sustainable programs and services that are community-focused. Her approach integrates research-based strategies to develop impactful initiatives that address the unique needs of diverse populations. Margo is deeply involved in evolving the programs she has co-developed, ensuring they remain effective and relevant in an changing healthcare landscapes. Currently, she is actively engaged in a study evaluating the impact of an oral health education program embedded in an acute care in-patient setting.
