On Thursday, June 11, The Change Foundation released Patient and Family Engagement in Primary Care: Building effective patient and family advisory councils in three Ontario communities,a case study report featuring 3 Family Health Teams. The report finds twenty key lessons in 5 categories- launching, recruiting, operating, relationships and sustaining.

Overview: Although foreign-born older adults (immigrant seniors) are at a higher risk for certain chronic diseases and negative health conditions, limited Canadian studies have examined healthcare experiences of foreign-born older adults (FBOAs), and few have developed or proposed practical solutions to support patient engagement, system navigation ,and care coordination.
Theme Description: Patients and caregivers are increasingly looking to be engaged and consulted in their own care. Primary care is finding innovative ways to support patient decision-making about their care and support for self-care. Presentations in this stream will include topics such as education programs for patients and their families; patient involvement in care planning; tools and coaching for patients to manage their own care; and using patient feedback to achieve a seamless patient experience.
