AFHTO’s “Bright Lights” Awards recognize AFHTO members’ leadership, outstanding work and the significant progress being made to improve the value interprofessional primary care teams across Ontario deliver.

Couchiching FHT to be named 2016 LEADing Practice

Congratulations to the Couchiching Family Health Team for being named a 2016 LEADing Practice Initiative. The FHT is being recognized for exemplary use of digital tools to strengthen clinical practice and provider experience. Couchiching FHT’s project integrated a tablet-based system into their EMR to screen patients at risk for chronic obstructive pulmonary disease (COPD).

NPAO honours AFHTO members’ nurse practitioners at annual conference

On September 25, 2015 the Nurse Practitioners’ Association of Ontario (NPAO) held their Awards and Celebration Dinner as part of their annual conference. Nurse practitioners (NPs) from different organizations were honoured, with half of the awards presented to NPs from member FHTs and NPLCs.

Championing Innovation in Quality Improvement Decision Support

The Association of Family Health Teams of Ontario (AFHTO) is pleased to announce the six recipients of Quality Improvement Decision Support (QIDS) Innovation Funds. Ontario Family Health Teams (FHTs) are committed to measuring and improving comprehensive primary care performance; the QIDS program is a provincial initiative to support AFHTO members in their quality improvement (QI) efforts.
