Learning Objectives

  • Learn an innovative approach to support patients with Mental Health and Addictions, building unique partnerships and having system-wide impact. Mobile Integrated Healthcare (MIH) uses a coordinated approach to patient-centered, evidence-based care that aims to divert low acuity 911 calls from ED to more appropriate support. Hear from a FHT Mental Health Nurse and an Advanced Care Paramedic how they respond to these calls and the impact of the team’s approach.   Key Learning will include: 
  • Identifying the power of system integration on streamlined referrals, appropriate service use and more 
  • Techniques for building and maintaining successful partnerships 
  • Demonstrated impact of MIH for ED diversion

Summary/Abstract

Unsustainable demands on emergency medical support and costly hospital EDs visits create a vital opportunity for innovation. Mobile Integrated Healthcare (MIH) is both a ground-breaking and practical approach to Mental Health & Addictions that improves patient experience by enhancing care tailored to individual needs and increasing access to the services they require. Rather than engaging in the cycle of confusion and unmet needs driving inappropriate ED use, MIH partnerships can shift the focus to provide the right care at the right time. The MIH model is successful in the UK and now in Niagara where hospitalization rates for mental health are high. We will share our impact to date, including the volume of calls and the reduction in ED use.  After a full year of implementation, results are impressive.
