Summary/Abstract With increasing pressure to open FHT programs & services to the community of non-rostered patients within a specific geography, this workshop will foster an understanding of SETFHT’s regional efforts under six (6) different models that enable FHT access to non-rostered patients. Taking an incremental approach to opening up FHT programs & services to the community provides a framework for others to follow in offering scalable outside access. Attendees of this workshop will learn practical strategies and approaches that help to identify where FHT programmatic capacity currently exists in opening up to the community within their own FHT(s), while ensuring that their rostered patients continue to receive the same level of access to high quality care following the implementation of opening up to the community. Additionally, attendees will work through which of the six (6) identified community access models provide the most value add to apply in a tailored approach to FHT program planning based upon; the assessment of non-rostered patient needs & regional needs; examination of existing in-house patient volume; understanding of organizational programmatic capacity; and the compliment of IHPs & physicians.
