Learning Objectives Participants will learn from our FHTs' "mistakes". Participants will learn from stand out examples how our FHTs' pre-op clinic, falls prevention programs and med rec encountered logistical difficulties; not proceeding as originally planned. Most importantly, participants will learn about the consultation and policy process, course corrections and data-based solutions undertaken to make improvements, re-direct resources, and/or re-launch to ensure enhanced patient outcomes. Also, importantly, in addition to learning best/worst practices in specific health program areas, participants will gain the skills, confidence and knowledge-of-approach to discuss, share and learn from "mistakes" in positive, constructive ways. Feel that conference presentations focus a bit too much on success when failure is supposed to be our best teacher? Want to learn from some "worst practices"? If so, this presentation is for you! With refreshing candour and bold truth-telling, come and here not about our family health teams "greatest hits" but rather our greatest misses. Come hear about good-sounding ideas that the data has nixed; programs initiatives that have had little-to-no uptake; and health care interventions that simply haven't worked. Confident in our FHTs' successful work and excellent results for patients, we will help your FHT learn from our mistakes in a safe environment of trust and non-judgment. Most importantly, participants will learn approaches and interventions to possibly avoid and/or how to maximize effectiveness building on the past work and lessons of others. Valuable course corrections, re-vamped successful alternatives, and concrete ideas to prevent failure, duplication and non-engagement will also be shared. We look forward to the discussion period.
