Learning Objectives Participants will learn about a New Primary Care Provider Orientation Program launched in the South West LHIN. Specifically, information on how the program was established, the process and outcomes will be shared. Participants will learn how this program can align with and be supported by FHT programs and processes. Summary/Abstract Interprofessional primary care teams combine the expertise of a range of health professionals to provide comprehensive primary care. Collaboration with system partners is essential to meet the needs of complex patient populations. Within FHTs, new primary care providers are provided with an introduction to the supports available to their patients. However, they are often not aware of programs and professionals available in the larger community. To address this need, an orientation program for new physicians and nurse practitioners (NPs) providing comprehensive primary care was developed by Partnering for Quality, a South West LHIN regional program in partnership with HealthForceOntario and other key stakeholders. An initial half-day orientation session is tailored to the participant’s specific needs. Presentations by the LHIN Sub Region Clinical Lead, local municipalities, health units and community support services provide content relevant to the provider’s practice and patient needs. If physicians and NPs are affiliated with a FHT, the site coordinator or appropriate administrators are also invited. This allows coordination of resources to ensure optimal use of both FHT and community based resources. Additional sessions are organized throughout the year to provide a comprehensive overview of support services not covered in the initial session. A Practice Facilitator is also available for ongoing support to provide clinicians with the tools and expertise required to improve practice efficiency and quality care. The support focuses on effectively accessing digital tools and improving ease of referral to community resources. Awareness of these resources reduces duplication and frustration with navigating the healthcare system. Presenters
