Learning Objectives

From the presentation, attendees will: 

  1. Increase their understanding of how to extend access for their patients to have virtual visits outside of the clinic setting with their physician, nurse practitioner, specialist or other members of the team. 
  2. Increase their knowledge of the variety of digital health options available to give fast access to patient-centre care. 
  3. Increase their knowledge of how to adapt the use of digital health options that work for patients with different digital literacy levels.

Summary/Abstract

Our presentation will capitalize on the real experiences of a rural FHT’s nurse practitioner, physician and telemedicine RN to demonstrate various digital health offerings that are being used in the FHT to provide patient and family-centred care beyond our clinic setting.  Specific examples of digital services that will be discussed include:  eConsult, eVisit, Virtual Visit, and Teledermatology. Practical information about what the services are, how to get started and key factors that will helped to ensure success will be reviewed. Additionally, we will cover, physician remuneration, overcoming barriers and how to adapt digital offerings that work for patients with different digital literacy levels. Our presentation will show various ways we have integrated communication modes outside “the norm” to better meet patients’ needs and expectations.    We know that improving our digital health capabilities in our FHT is a growing expectation of our patients. By successfully embracing digital health in our FHT and utilizing resources, patient satisfaction can be improved by creating workable alternatives to in-person care.  We are currently documenting patient feedback and looking at capturing cost efficiencies of using more digital health options in our clinic. Finally, the impact on increasing the capacity of our front-line providers to efficiently get the information they need and communicate that in a timely manner with their patients is significant.
