Summary Abstract:

Health systems globally are striving to transform primary care services to deliver improvements across IHI’s Quintuple Aim Framework  (improving population health, enhancing the care experience, reducing costs, addressing clinician burnout, and advancing health equity). However, for large-scale transformation of this nature to succeed, it must be grounded in the actual needs of stakeholders. That means putting humans — including staff, patients, families, and caregivers — at the heart of a strategy that is equal parts innovative and practical, and doing so now. This interactive session will provide an overview of primary care network (PCN) transformation experiences in other jurisdictions, including Ireland, highlighting the similarities and differences between these systems and Ontario’s health system.  It will discuss the change journeys from policy decision through to current state implementation, the target objectives and the outcomes achieved to date. The presentation will discuss key topics including access to team-based care, management of chronic disease, integrated care delivery across acute and community settings, physical infrastructure, and funding. A robust question and answer period will provide attendees with an opportunity to explore topics of interest in further detail.
