The Georgian Nurse Practitioner-Led Clinic (GNPLC) is a multi-site primary care clinic in Barrie and Innisfil. Our team is comprised of nurse practitioners, registered nurses, registered dietitians, social workers and occupational therapists and supported by an administrative team. Social Worker
Social workers assist individuals, families and groups in dealing with complex social issues that may be affecting their overall health. As members of the interdisciplinary team, you will provide short-term counselling and patient-centered clinical assessment, psychoeducation, skills development, and resource navigation within individual and group settings.
Responsibilities
- Conduct psychosocial assessments
- Provide individual, couples’, family, and/or group counselling to a diverse patient population
- Provide crisis intervention, monitoring of symptoms, education, advocacy, group work, life skills training, and/or clinical case management based on the needs of the patient
- Develop and implement appropriate treatment plans in collaboration with patients and providers
- Collaborate and consult with GNPLC team regarding patients, as needed
- Maintain clinical records in accordance with professional practice standards
- Connect and refer to services and agencies in the community to best support patient needs
- Work in collaboration with agencies/services in the community and family members, wherever appropriate, for the purpose of providing the best care possible to patients
- Contribute to and participate in program/service planning and evaluation
- Assume other duties that fall within the scope of practice for social work, as directed by the Executive Director and/or Clinical Lead
Qualifications
- Commitment to diversity, equity, and inclusion
- Master of Social Work (MSW) from an accredited university
- Three years' experience in general clinical practice (individual, couple, family, group therapy)
- Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Experience using evidence-based mental health treatment approaches in a short-term counselling setting e.g. CBT, DBT, ACT
- Independent decision making and professional judgment on matters pertaining to problem assessment, methods of intervention, and patient safety. Requires strong critical thinking, analysis, and/or reasoning
- Ability to deal effectively with crises
- Experience working with individuals of all ages with a range of mental health and life challenges including but not limited to depression, anxiety, trauma, housing instability, chronic disease, substance use issues, ADHD, ASD, grief/loss, and emotional dysregulation
- Proficiency with Accuro EMR and Microsoft Suite of products
- Knowledge of community systems and patient resources in Simcoe County
- Excellent communication, facilitation, and organizational skills
- High degree of resourcefulness, flexibility, and adaptability
- Commitment to confidentiality
