Job Description

The ELFHT provides primary health care, including chronic disease management, health promotion, and disease prevention, to a rostered and unattached population of patients in the local community and surrounding area. It employs an interprofessional team of health care providers who work in a collaborative model of care.

The ELFHT is seeking a 1.0 FTE Social Worker (37.5 hrs/week) to join its growing team of healthcare professionals. The position is for 5 days a week.

You will work independently and in collaboration with our multidisciplinary team to provide excellent mental health care to various patients with a broad spectrum of presenting needs. While carrying a clinical caseload, you will also work with health providers to meet patient needs in the clinic, triage physicians, make interdisciplinary health referrals to the FHT mental health program, and assist patients in accessing appropriate services elsewhere in the community.

You will join our FHT in collaborating with community partners to creatively and effectively meet the mental health needs of our patients and individuals in the wider community.

The ideal candidate will have a Master’s Degree in Social Work, 2-3 years of clinical experience, be a member in good standing with the Ontario College of Social Workers and Social Service Workers and possess valid professional liability insurance.

QUALIFICATIONS

  • Excellent clinical skills and experience with assessment, treatment and management of mental health problems commonly seen in primary care
  • Ability to work with a team and the community to create optimal mental health care plans
  • Systems navigation experience and working knowledge of a wide variety of community resources relevant to mental health and socio-economic challenges
  • Knowledge of the Mental Health Act and other relevant legislation
  • Experience with group facilitation
  • Knowledge of DBT, CBT and ACT models
  • Specific experience with a geriatric population is an asset
  • Knowledge of electronic medical records (EMR) is an asset
  • Knowledge of patient privacy and confidentiality as per PHIPA
  • Ability to speak and write in English and French, preferred
