Your Opportunity:

We are currently recruiting for a full-time Registered Practical Nurse (RPN) to join our Interprofessional Primary Care team. This innovative role will support patient attachment to primary care, promote attendance at groups and support the organization of community events. From a clinical perspective, the role includes TB skin testing, clinics to provide outreach to those that do not have access to primary health care, and support to primary clinical care. Additionally, you may be assigned to work on other small projects throughout the organization within your scope of practice.
 

A Successful Candidate in This Role Will Have:

  • Registered Practical Nurse diploma.
  • Certificate of registration from the College of Nurses of Ontario.
  • Three to five years of nursing experience in a community setting or a combination of community, hospital, or public health settings.
  • Experience in primary care is an asset.
  • A caring approach to care when supporting patients and their caregivers.

Responsibilities:

Attachment

  • Support patients to access primary care ensuring warm transitions and collaboration with other community partners.
  • Maintain statistics for attachment of patients.

Groups

  • Promotion of groups and workshops to patients and partners.
  • Potential to facilitate and build workshops for patients.

Community

  • Participate in community events where HFHT will support individuals to find family doctors and promote attendance at HFHT groups and workshops.

Clinical

  • Responsible for TB clinic for HFHT patients including testing and assessment.
  • Support primary care at Eva Rothwell or other clinics as needed.
  • Assist with the organization and implementation of clinics.

Patient Experience:

  • Support patients and their caregivers, ensuring their questions are answered and a seamless care approach is in place.
  • Liaise with other care team members and partners to provide a patient-centered approach.
  • Quality and Performance Improvement:
  • Maintain statistics for tracking the attachment of patients to primary care.
