Couchiching Family Health Team
Position: Registered Social Service Worker, Trans Health Services
(1.0 FTE Position, Contract until March 31, 2026, no benefits)
Reports To: Manager, Specialized Programs & Services
Position Summary:
The Registered Social Service Worker (RSSW) will provide comprehensive care for clients and their families/loved ones within the Trans Health Services (THS) Program. The RSSW will communicate and collaborate with other members of the Trans Health Services Team in the provision of evidence-based care and connections to community supports and resources.
The RSSW will be expected to understand barriers and issues faced by gender diverse individuals and their families/loved ones. The RSSW for THS will establish a network within the community creating links between social services and health care agencies, also addressing social determinants of health for this population. The RSSW for THS will be involved with patient and caregiver interviews, current and future state mapping, data analysis and reporting.
Roles and Responsibilities:
- Support gender diverse clients and their families/loved ones with social determinants of health
- Identify service gaps and duplication in the community
- Communicate goals of Trans Health Services team in the community
- Facilitate psychoeducational group programming within THS
- Create links within the community with ODSP, OW, and community housing
- Complete required forms to support client goals
- Prepare reports and monitor daily statistical data
- Collaborate and/or consult as appropriate, with members of an interdisciplinary team within the clinic and externally with the community of care
- Provide assessment of goals, linking with community partners, advocate for clients and manage social issues that are barriers to care
- Participate in committees or community groups where an identified need for advocacy or representation is required
- Sit on core working groups as needed
- Provide culturally sensitive care
- Provide services that reflect excellent gender-awareness and acceptance
- Conduct in-services and workshops for team members, other health professionals and community partners as appropriate
- Assist with Home Visits when needed by adhering to the Home Visit Policy Guidelines
Additional Roles and Responsibilities:
- Maintain current knowledge of policies and procedures
- Manage personal health and social information in accordance with privacy laws and ethical standards.
- Maintain confidentiality in accordance with the Municipal Freedom of Information and Protection of Privacy Act.
- Work in compliance with Health and Safety laws adhering to safe work practices and procedures established by the CFHT
- Other duties as required
Skills/Knowledge Requirements:
- Exceptional understanding of current, evidence based and best practice guidelines
- Maintain and upgrade knowledge and skills
- Excellent communication and organizational skills
- Demonstrated ability to work independently and in a team environment
- Knowledge of community assessment and development
- Exceptional knowledge of the barriers and issues that gender diverse individuals and their families/loved ones face daily
Education/Specific Job Requirements:
- Degree or Diploma in Social Service Work, and registration with the College of Social Workers and Social Service Workers of Ontario
- Additional training or credentialing is considered an asset.
- High sensitivity to psychosocial, social, mental health and health needs of gender diverse clients.
- Exceptional knowledge of social service system/resources.
- Certification in CPR (BLS-C)
- Current Ontario Driver’s License
- Able to work flexible hours including evenings if required
- Ability to travel within the community
- Police Record Check
- It is a condition of employment that you receive the necessary vaccinations to protect against infection of COVID-19, or any similar infectious diseases, which may be recommended by governmental and public health authorities from time to time
Experience:
- 1 to 2 years related or applicable experience in a community setting, preferably working with the gender diverse population
- Experience working with ODSP/OW, housing, and providing case management is required
- Experience with Practice Solutions an asset
- Priority will be given to those who identify as part of the LGBT2SQ+ population
