Job Description:

The Registered Nurse (RN) is an integral part of our interdisciplinary team and vital to the health and well-being of the community we serve by providing a range of primary care services to individuals of all ages. Operating within their full scope of practice, the Registered Nurse provides patient care and supports the delivery of programs by focusing on the identification, provision and management of resources and self-management strategies to improve patient health.


Responsibilities:

  • Observes and records vital and biometric measurements as well as outcomes;
  • Triages patients according to acuity;
  • Ensures immunizations, supplies and equipment inventory is monitored and ordered for daily patient care and emergency situations;
  • Performs various clinical procedures such as suture removal, skin and wound dressing, TB testing, wart treatment, ear syringing and assists with minor procedures;
  • Administers various vaccines in different ages and other injections (B12, Prolia, Testosterone and Anti-psychotic drugs);
  • Ensures immunizations are current and educates patients as to their need and any possible side-effects;
  • Partners and works collaboratively as part of an interdisciplinary team;
  • Contributes to development of organizational policies, workflow processes and medical directives
  • Participates in quality improvement initiatives including planning, implementation and reporting;
  • Provides individual or group patient education and contributes to the development, implementation and/or evaluation of programs offered at SFHT that address prevention or management of chronic illness;
  • Maintains accurate, concise and confidential EMR documentation;
  • Maintains the INR monitoring and management system as per guidelines;
  • Sterilizes equipment for minor surgical procedures;
  • Completes all appropriate forms as required;
  • Other duties as assigned.


Qualifications:

  • Currently registered in good standing as a Registered Nurse (RN) with the College of Nurses of Ontario with evidence of current malpractice liability insurance;
  • Sound knowledge of primary care nursing processes, theory, and responsibilities to the nursing profession;
  • Excellent patient assessment and triaging skills;
  • A minimum of 2 years working experience in a primary care setting;
  • Excellent communication skills;
  • Compassionate and sensitivity to patient needs and confidentiality/privacy is a must;
  • Problem solving skills, ability to use sound judgement;
  • Interpersonal skills, diplomacy, and discretion, excellent telephone manner;
  • Excellent oral and written communication skills;
  • Ability to manage and prioritize tasks;
  • Organized, accurate and detail-oriented;
  • Trained in Electronic Medical Records, Accuro is an asset;
  • Additional certifications and trainings are an asset, e.g. Diabetes Educator, Respiratory Educator, Spirometry and Prescribing;
  • CPR certification required;
  • Able to work flexible hours in order to meet organizational needs which may include evenings and weekends
