Job Description
The North Simcoe Family Health Team (NSFHT) provides education and support services to patients
with chronic illness. We strive to promote healthy living and prevention of complications by enhancing
self-management practices and care.
The Registered Nurse in the diabetes education program has taken specialty courses in diabetes
education, prevention and management of disease. The diabetes nurse educator is an active member
of a multidisciplinary team, providing education for the prevention and management of diabetes, as per
current Clinical Practice Guidelines. They are instrumental in meeting the needs of the diabetes
community, utilizing the concepts of self-management, group visits and individual sessions based on
priority needs. The NSFHT is currently seeking candidates for the following position:
Diabetes Nurse Educator– Permanent 1.0 FTE
Salary $ 74,148.00 plus HOOP and Benefits
The successful candidate will possess the following qualifications:
- Baccalaureate of Science in Nursing required.
- Certified Diabetes Educator (CDE) with the Canadian Diabetes Educator Certification Board
(CDECB) an asset. - Minimum of five (5) years clinical nursing experience preferably with experience working as
a diabetes educator. - Current registration with the College of Nurses of Ontario.
- Basic CPR certification.
- Demonstrated experience in primary care and/or community setting.
