Position: Registered Nurse – Chronic Disease Management (1.0 FTE Permanent with Benefits)
Reports To: Chronic Disease Manager
Position Summary:
As a key member of the Couchiching Family Health Team (CFHT), and an integral part of an interdisciplinary health care team, The Chronic Disease Management Registered Nurse serves as practitioner, educator, consultant, and collaborator to provide advanced clinical and analytical skills in the care/management of our patient population. Conducts nursing research and performance improvement initiatives as assigned.
Your primary role is to work in partnership with the Family Health Team to help educate patients on chronic disease management with an emphasis on Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD). You will assess the condition of patients and participate in treatment plans and provide education to patients and their families and help them make lifestyle changes to treat their conditions.
You will deliver services to a variety of patient populations including high-risk groups suffering from chronic diseases and/or complex multi-system diseases. You will also provide health education to our patients with a focus on health promotion, disease prevention, and chronic disease management.
Roles and Responsibilities:
- Communicate to Specialists in the evaluation of patients when working in that program
- Communicate with specialists, Primary Care Providers (PCP’s), and Chronic Disease Manager
- Take direction and be accountable to specialists regarding individual patient care
- Communicate with the Chronic Disease Manager with issues relating to development of patient materials, scheduling and program development and setting standards for the program
- Develop a comprehensive individualized plan of care for patients, which includes teaching, discharge planning, and necessary referral to other members of the health care team
- Demonstrate critical thinking skills in clinical decision making by the identification, evaluation, and management of the individual needs of patients/families
- Determine the appropriate service or treatment with the appropriate provider
- Involve patients in decisions about their own health
- Collaborate with members of an interdisciplinary health team
- Handle patient phone calls, provide telephone advice and arrange for follow up
- Prepare monthly reports for Ontario Health as required
- Work in compliance with Health and Safety laws adhering to safe work practices and procedures established by the CFHT
- Maintain current knowledge of policies and procedures
- Maintain confidentiality in accordance with the Municipal Freedom of Information and Protection of Privacy Act
- Other duties as required
Skills/Knowledge Requirements:
- Forward thinker who can help move the programs into a new direction based on the needs of the patient and funding that is available
- Able to communicate with external stakeholders and demonstrate leadership qualities
- Demonstrated competence in electronic communication
- Knowledge of chronic disease and multiple morbidities
- Maintain and upgrade knowledge and skills
Education/Specific Job Requirements:
- Bachelor’s Degree in Nursing
- Current membership with the College of Nurses of Ontario (CNO)
- Current membership in professional associations: Registered Nurses Association of Ontario (RNAO)
- Current Basic Cardiac Life Support (BCLS) Certification
- Advanced Cardiac Life Support (ACLS) Certification will be considered an asset
- Access to reliable transportation for travel between sites may be required
- Police Record/Vulnerable Sector Check
- Able to work flexible hours including evenings if required
- It is a condition of employment that you receive the necessary vaccinations to protect against infection of COVID-19, or any similar infectious diseases, which may be recommended by governmental and public health authorities from time to time
Experience:
- Preferred experience working in a chronic disease program, within a clinic and/or community setting
