Job Summary
Summerville Family Health Team is looking for a full-time Registered Dietitian (RD). The Registered Dietitian is an integral part of our interdisciplinary team and vital to the health and well-being of the community we serve. While working closely with our interdisciplinary team, the RD will provide current, evidence-based nutritional information, including medical nutrition therapy and patient-centered nutrition care plans to a diverse population throughout the life cycle including those with or at risk for diabetes, heart disease and other chronic conditions. The RD will be comfortable working to their full scope of practice by focusing on both the provision of direct patient care as well as assisting in the development, implementation and evaluation of programs and resources.
 

Duties and Responsibilities 

  • Develop individualized patient-centered nutritional care plans based upon a comprehensive needs assessment, in accordance with standards set by the Dietitians of Canada and related Ontario regulations, SFHT guidelines and in collaboration with other SFHT providers;
  • Provide information and education to patients by coaching and guiding behavioral changes related to food choice, eating patterns and food preparation methods that promote self-efficacy and positive decision making to optimize health and well-being;
  • Provide group patient education and contribute to the development and implementation of programs offered at SFHT that address the areas of food and nutrition to prevent disease and promote health;
  • Participate in quality improvement initiatives including planning, implementation and reporting;
  • Engages with community partners to plan and/or deliver services with the goal of improving the health of people in our community (e.g. MOHT initiatives).
  • Maintain accurate, concise and confidential EMR documentation pertaining to patient care and ensure ongoing communication with the care team;
     

Qualifications and Skills 

  • Bachelors program completion with a major in Nutrition; 
  • Registered member in good standing with the College of Dietitians of Ontario, with evidence of current malpractice liability insurance;
  • Minimum two years of practical experience, ideally in a primary care environment;
  • Successful completion of an accredited dietetic internship program;
  • Experience with Motivational Interviewing, CBT and other types of counselling skills an asset;
  • Experience leading and implementing patient education and behavior change programs, such as CHANGE, Heart Health, Diabetes;
  • Health promotion skills including population health;
  • Strong health assessment and clinical skills with a patient-centered approach;
  • Able to partner, act as a resource and work collaboratively as part of an interdisciplinary team;
  • Good critical thinking and decision making skills;
  • Ability to use digital technologies for patient care and program delivery (e.g. Zoom); Intermediate knowledge of MS word, Excel and EMR (e.g. Accuro);
  • Excellent written and verbal communication skills, including facilitation skills;
  • Ability to adapt to a wide variety of patient populations and needs.
