Status: Full-Time Temporary to March 31, 2025, with significant opportunity for permanent placement
Wage: $50.80/hour

Position Overview:

The Pharmacist practices patient-centered care to achieve optimal drug therapy outcomes. In partnership with patients and the other health care providers of the North Durham Family Health Team. The Pharmacist uses unique knowledge and skills to prevent or resolve actual and potential drug related problems. Pharmacists are health care professionals who are medication experts.

The Pharmacist as part of the interdisciplinary team assists in primary care functions and participates in the development, implementation, monitoring and evaluation of programs and services of the North Durham Family Health Team for the individuals, families of the North Durham community.

Duties and Responsibilities:

  • Conduct patient medical histories and identify medication related problems such as adverse drug reactions, drug allergies and known contraindications to prescription, non- prescription, natural/alternative medicines, and develop appropriate care plans
  • Communicate care plans to patients, primary care providers, and other members of the interdisciplinary team
  • Provide consultation to the family physician and other interdisciplinary providers to assist in the individual care of the patient
  • Review patient medication lists upon discharge from hospital and complete medication reconciliation
  • Provide support to patients of other NDFHT programs such as Memory Clinic, Diabetes Education and Heart Function Clinic to ensure optimal drug therapies are achieved
  • In collaboration with other interdisciplinary providers, and based on the needs of the patients of the NDFHT, design, develop and deliver group patient presentations on specific therapeutic topics
  • Develop and deliver presentations to other interdisciplinary providers and physicians on various relevant therapeutic topics and other medication related procedures
  • Prepare monthly or statistical reports as required by the NDFHT
  • Actively participate in interdisciplinary, staff, team and committee meetings
  • Participate in NDFHT educational programs and relevant external workshops, courses and conferences
  • Contribute to and participate in overall NDFHT planning, programming and evolution.

Qualifications and Relevant Experience:

  • Completion of a Bachelor of Science in Pharmacy or PharmD program from an accredited university
  • Current Part A registration with the Ontario College of Pharmacists
  • Hospital or Primary Care residency is an asset
  • Previous experience in primary patient care is an asset
  • Additional training in pharmacotherapy e.g. Certified Geriatric Pharmacist, Certified Diabetes Educator, Opioid Dependence Certificate etc. an asset
  • Professional liability insurance

Skills/Abilities:

  • Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction.
  • Superior leadership, organization, research, evaluation, time management, communication and interpersonal skills.
  • Proficiency in the use of computer hardware and software, particularly in Microsoft Word, Excel, and Outlook.
  • Prior experience and knowledge of electronic medical records.
  • Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences.
  • Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating a patient.
  • Excellent verbal and written communication skills.
  • Exhibit an ability to be open and non-judgmental.
  • Knowledge of the goals and structure of the organization, the policies and procedures and the programs.
  • Demonstrated ability to welcome change and manage it innovatively
