Job Description:

The Kincardine Family Health Team is seeking an Occupational Therapist (OT) to join our team of interdisciplinary health care providers.

The Kincardine Family Health Team (KFHT) is a group of allied healthcare professionals with a mission to provide comprehensive patient care utilizing an interdisciplinary approach that is responsive to the needs of the community. As an Occupational Therapist with the KFHT, you will have a passion for geriatric care, health promotion, and program development.

Roles and Responsibilities:

  • Provide individual assessment and interventions focussed on improving or maintaining function, promoting health, supporting caregivers, and system navigation
  • Collaborate with members of the interprofessional primary care team (family physicians, nurse practitioners, nurses, dietitian, social worker, kinesiologist, respiratory therapist) to deliver comprehensive care and service coordination
  • Collaborate with partner organizations as appropriate to address patient goals and promote smooth transitions of care.
  • Maintain complete and accurate electronic medical records
  • Actively participate in program planning, quality improvement initiatives, and meetings as required
  • Maintain professional and technical knowledge

Programs and Services:

  • Involvement with several programs delivered by our team, including, but not limited to:
  • General Occupational Therapy Assessments for adults and older adults
  • Home Assessments/Visits
  • MINT Memory Clinic
  • Fall Prevention
  • Pain Management
  • Mobility Device Assessments
  • Design and administer individual and group programs, both in person and using virtual platforms
  • Act as the lead or coordinator for interdisciplinary programs, subject to scope and experience
  • Provide mentorship and skills training for co-workers and students

Education / Specific Job Requirements:

  • Graduate of an accredited school of Occupational Therapy
  • Current registration with the College of Occupational Therapists of Ontario and evidence of professional liability insurance
  • Vulnerable Sector Screen
  • Membership with a professional association (OSOT or CAOT) preferred

Experience: 

  • 2-3 years of experience in a health care setting preferred, with experience in primary health care, community health care, and/or hospital setting
  • Experience with geriatric care and caregiver teaching and support
  • Strong clinical and case management skills related to establishing, setting, treating, and monitoring patients’ functional goals and facilitating transitions in care 
  • Proven efficient and effective patient care and ability to leverage existing resources when available
  • Experience in program development, implementation, monitoring, and evaluation
  • Ability to exercise sound clinical judgement and to function in an independent, self-directed manner
  • Proficiency in the use of EMR software applications and virtual communication platforms
  • Commitment to working from a community- based and health equity perspective
  • Excellent communication, organizational, administrative, and interpersonal skills
  • Vestibular Assessment experience is an asset
