Job Description:

Mississauga Ontario Health Team is partnering with Summerville Family Health Team, Credit Valley Family Health Team and Trillium Health Partners Women’s & Children’s Program, to implement a temporary community-based Paediatric Care Clinic. This clinic will focus on alleviating emergency department pressures by diverting paediatric patients, with certain non-urgent care needs (e.g. cough, cold and flu symptoms) to our Paediatric Care Clinic in Mississauga. Summerville Family Health Team will be responsible for hiring and managing staff on a day-to-day basis. The clinic will be located at one of Summerville’s sites in Mississauga, Ontario.


Job Summary:

We are seeking a part time Nurse Practitioner (NP) for a short-term contract until June 30th, 2024, with the potential for extension. The Nurse Practitioner will be an integral part of our temporary Paediatric Clinic, providing a range of primary care services to individuals between the ages of 6-months to 17 years old. The NP must be comfortable working independently to their full scope of practice.


Duties and Responsibilities:

  • Provides comprehensive and excellent health care to patients, in accordance with standards set by the College of Nurses of Ontario (CNO), and in collaboration with other team members in a patient-centered manner;
  • Completes assessments, diagnoses and provides holistic care for our paediatric patients;
  • Plays a cooperative role within the Clinic by collaborating with other team members; and working closely with the Physician lead of the Paediatric Clinic;
  • Identifies and implements quality improvement initiatives to increase the efficiency and/or effectiveness of patient care;
  • Maintains accurate, concise, and confidential EMR documentation pertaining to patient care and ensures ongoing communication with the care team.


Qualifications and Skills:

  • Master’s Degree in Nursing from a recognized Post-Secondary Institution;
  • Minimum of 3 years working experience in a primary care setting;
  • Knowledge and comfort in working with young children;
  • Current registration with the College of Nurses of Ontario as a Nurse Practitioner – Primary Health Care;
  • Liability Protection through the Canadian Nurses Protective Society (CNPS);
  • CPR Certification required;
  • Intermediate knowledge of MS word, Excel and EMR (e.g. Accuro);
  • Able to partner and work collaboratively as part of a multi-disciplinary team;
  • Excellent written and verbal communication skills, including facilitation skills.
