Overview

The Erie Shores Family Health Team (ESFHT) provides primary care to more than 30,000 patients in the Leamington, Kingsville, and South Essex County area. ESFHT has multiple locations with family physicians, nurse practitioners, nurse educators, social workers, a dietitian, health promoter, chiropodist, pharmacist, and administrative staff. The Nurse Practitioner is responsible to provide primary care services to patients at the clinics or in their homes.

Job Responsibilities

  • Provide primary health care services including assessments, diagnosis and treatment, health education and counselling, and other clinical activities according to the College of Nurses of Ontario standards of practice for Registered Nurses in the Extended Class.
  • Consult with other members of the healthcare team as needed, and work in partnership with other team members to share responsibility for the planning, provision, and coordination of patient care.
  • Work within medical directives as established and approved by the ESFHT.
  • Collaborate within a primary health care framework that emphasizes disease prevention, health promotion, and education.
  • Maintain complete and accurate medical records using the electronic health record (Accuro), participate in chart reviews and case conferences as appropriate.
  • Participate in the growth of ESFHT by providing input into program development, delivery, and evaluation, participating in team events and other committees, and networking with other institutions, organizations, and health and social service professionals.
  • Contribute to the education of nurses and other health professionals through teaching and presentations, as well as facilitate own professional development through formal education and/or mentoring opportunities.
  • Perform other duties that support the ESFHT mission and vision as assigned based on position responsibilities and/or site requirements.

Requirements

  • Member in good standing in the Extended Class with the College of Nurses of Ontario.
  • Certificate in Primary Care for Nurse Practitioner preferred.
  • 2-5 years primary care experience preferred.
  • Experience with Accuro EMR an asset.
  • Demonstrated ability to work effectively in a multi-disciplinary environment.
    Demonstrated commitment to and knowledge of community-based health care.
  • Awareness of and sensitivity to the health issues of a diverse community.
  • Excellent communication, interpersonal, organizational, and administrative skills, including computer literacy.
  • Multilingualism an asset (Spanish, Low German, and/or French).
  • Current First Aid/CPR required.
  • Current Police Clearance with Vulnerable Sector Search required.
     

Additional Information

  • 37.5 hours per week, Monday to Friday during regular business hours.
  • Primary care patient load varies from 12-18 patients per day.
  • $63.91/hour with full benefits and 5% retirement plan contribution.
