Overview

Nurse Practitioner 

Full-Time Contract 

Reporting to: Executive Director 

The Burlington Family Health Team (BFHT) is a dynamic primary care organization that provides team-based primary care and programing including psychotherapy, physiotherapy and remote care management for chronic disease, designed to improve the health and well-being of people living in the community of Burlington. 

The BFHT is expanding its programs in Burlington, with a focus on improving access to interdisciplinary primary care for frail older adults, individuals who are precariously housed or at risk of mental health and addictions, and individuals who are uninsured or having difficulty getting attachment to a family doctor. The BFHT is seeking an enthusiastic and talented Nurse Practitioner to be a key member of an expanded Interprofessional Primary Care Team (ICPT) that will provide assessment, intervention and education through target-specific clinics and primary care provided mostly through a community outreach model. 

Position Summary:

Operating within their full scope of practice, the Nurse Practitioner will lead the primary care services offered through an Outreach Clinic to be held at various community locations and facilitate transition to other services, as appropriate. Additionally, the Nurse Practitioner is an integral part of the BFHT interdisciplinary health care team supporting people through a memory clinic, frailty screening clinic and health promotion and education activities. 
The position provides in-person clinical care Monday to Friday. Occasional after-hours and weekend hours may be required to meet program needs. BFHT is committed to supporting work/life balance for our team members. 

Responsibilities:

Primary Role 

  • Work collaboratively as a member of a high functioning interdisciplinary primary care team providing patient-centred care 
  • Lead clinician for the expanded Interprofessional Primary Care Team (IPCT) programming
  • Provide in-person assessments, diagnoses and treatments for acute and episodic illnesses and chronic disease management within the full scope of NP practice
  • Apply recognized clinical practice guidelines and best practices to screening, monitoring, and assessment of patient’s physical and emotional well-being
  • Demonstrate a high level of autonomy and expert skills in formulating clinical decisions and managing acute/chronic illnesses
  • Provide educational support to patients, families and/or caregivers 
  • Assume leadership role in the NP-led primary care outreach clinic and appropriately manage their own roster of patients as well as non-rostered patients
  • Participate in the development of expanded IPCT clinic practices and procedures
  • Occasional after hours coverage to meet patient needs
  • Ongoing program development and evaluation

Qualifications:

  • Current certificate of competence with the College of Nurses of Ontario as a Nurse Practitioner, preferably Primary Health Care
  • Current CPR Certification 
  • Provision of care, supervision and documentation to CNO practice standards
  • Demonstrated ability to assist patients, family members or other clients with concern, compassion and empathy, respecting their confidentiality and privacy and communicating in a courteous and respectful manner
  • Solid understanding of social determinants of health and the associated barriers to accessing care
  • A well-defined sense of diplomacy, including solid negotiation, conflict resolution, and people management skills
  • High degree of resourcefulness, flexibility and adaptability 
  • High level of critical and logical thinking, analysis, and /or reasoning to identify underlying principles, reasons, or facts 
  • Knowledge of and adherence to the Personal Health Information Privacy Act
  • Demonstrated ability to work well independently and as a member of an interdisciplinary team
  • Ability to collaborate with health care partners to address barriers to access 
  • Participate in continuing education activities to maintain and develop scope of practice 
  • Valid Ontario driver’s licence and access to a vehicle to provide services through community outreach and home visits. 
     

Education:

  • Masters degree - Nurse Practitioner Primary Health Care (NP-PHC)

     

Experience: 

  • Minimum 3 years of recent experience as a primary care nurse practitioner 
  • Experience working with older adults and equity-deserving populations 
  • Experience in Quality Improvement, program development and monitoring key performance indicators
  • Experience working in a team based, interdisciplinary clinic setting, preferably a Family Health Team 
  • Familiarity with comprehensive geriatric assessment and related tools
     

The work environment for this position involves providing in-person services in a variety of community settings (including but not limited to primary care clinic, a mobile clinic, a shelter, community locations etc.) The applicant must have access to a vehicle and is reimbursed for mileage. Weekend and/or on-call coverage may be required.
