Job Summary:
Reporting to the Executive Director, the Nurse Practitioner and Clinical Supervisor will co-lead the design and operation of a community clinic for acute and episodic care for unattached patients. This role includes providing direct care, supervising Nurse Practitioners, Registered Nurses, and Registered Practical Nurses at our Brighton and Quinte West sites and overseeing various health programs. The position requires close collaboration with the Executive Director on program planning and delivery methods to ensure alignment with our organizational goals.

Salary Range: $105,000 - $118,000

Key Responsibilities

  • Clinical Duties: Conduct assessments, diagnoses, treatments, and health education/counselling. Manage patient care and treatment plans. Provide direct and indirect chronic disease care, screening, and health promotion. Maintain accurate medical records and consult with collaborating physicians.
  • Administrative Duties: Organize appointment schedules, manage referrals, and maintain up-to-date patient documentation. Participate in program planning and development.
  • Supervisory Duties: Foster a positive organizational culture. Lead by example, support internal communication, monitor training and development, set performance goals, conduct reviews, and ensure adherence to relevant legislation.
  • Organizational Duties: Participate in team activities, events, and committees. Maintain appropriate boundaries and attend required meetings and training sessions.
     

Qualifications

  • Education: Current registration with the College of Nurses of Ontario as an RN (extended class). Membership in RNAO and NPAO is an asset. NP-Primary Health Care specialty certificate. Master’s degree and diabetic educator certification preferred.
  • Experience: Proven experience in primary health care or community settings, working with individuals facing barriers to healthcare. Leadership experience in team settings.
  • Skills: Strong verbal and written communication, conflict resolution, and negotiation skills. Ability to work independently and as part of a multidisciplinary team. Knowledge of community resources and a commitment to an integrated model of practice. Strong time management, prioritization, and innovation skills.
