Learning Objectives

Participants will learn how to implement a team-based approach to COPD management for patients with comorbid serious mental health diagnoses and, often, low health literacy.  Ms. Lisa Smyth, Nurse Practitioner Village FHT will lead the presentation. Innovative low literacy tools will be shared, designed by Village FHT, to reach this challenging patient population.    

Summary/Abstract

The presentation will outline the process through which the COPD program was developed at Village FHT. They will review:

  • Initial needs assessment for the program 
  • The creation of a chronic disease management tool to standardize evidence- based quality care for all COPD patients.
  • Development of low literacy health teaching tools with input from patients to facilitate individualized self-management plans.
  • Alignment with HQO COPD Quality Standards including introduction of on-site spirometry
  • Outcome measures
  • Areas for future development
  • Challenges and successes along the way!
