Summary/Abstract

Nurses support patients in family health teams and are often the first point of contact. When working to their full scope and alongside a wide range of interdisciplinary healthcare providers, programs, and services, they are able to deliver comprehensive care which includes wellness and prevention, chronic disease management, diabetes care, mental health, and system and care navigation and coordination. The updated handguide, now inclusive of RPNs and enriched with additional details and a summary of relevant guidelines, serves as a valuable reference to help nurses develop and refine the competencies, enabling them to act more autonomously and with greater confidence. By consolidating this information into a single document, nurses can increase efficiency and time management, allowing them to focus on delivering direct patient care.
