Meaningful Measurement in primary care: Beyond the body parts

NB: Personal invitations have already been sent to all LHIN primary care and quality leads. The information below is being shared with AFHTO members for use in encouraging their LHIN physicians leads to attend the session. The invitation is limited to the LHIN physician leads.  You are invited to join your peers to learn more about meaningful measurement in primary care and what it could look like to move measurement beyond the body parts.  Join us from 6:00-8:30 pm on April 4, 2017 for dinner at the Faculty Club of the University of Toronto to hear:
  • Dr Danielle Martin - talk about “Big Idea #1” for making the healthcare system “Better Now” – ie the importance of the relationship between patients and their primary care providers.
  • Dr Rob Annis and Dr Sean Blaine - talk about how primary care teams are measuring what matters and delivering good quality at lower healthcare SYSTEM costs – and more importantly, how everyone else in Ontario can do it too
  • Darren Larsen - talk about help for all primary care providers in using their EMRs to make their own lives and those of their patients better now
There is no charge for the event and we are offering an honorarium to recognize your time/effort in attending. Contact Carol Mulder for more information.
