The submission platform for D2D 5.1 is open; it will close on February 8th, 2018. The Interactive report will be launched March 8th, 2018.
Data to Decisions (D2D) is a membership-wide report on performance in primary care. This tool allows you to enter and review your own data and compare it to data from your peers. As always, D2D is a come-as-you-are party; you can enter data for as many (or as few) indicators as are manageable and meaningful to your team.
What’s new?
D2D 5.1 is identical to D2D 5.0. This will allow teams who participated in both iterations to do 1:1 comparisons of their results. Here are the changes we made in D2D 5.0, to help our members get a clearer picture of what enables success in quality improvement:
- To enable direct peer-to-peer comparison, you have the option to unmask yourself to your peers.
- You can report your LHIN sub-region, which will allow comparison within and across sub-regions. Not sure which one you’re in? Look it up here, or contact us for help.
- By answering team profile questions, you’ll get a clearer picture of what steps you can take as a team to make your quality improvement efforts more successful.
- We made follow-up after hospitalization a core D2D indicator.
- We added a new exploratory indicator about individualized HbA1C targets recorded in the EMR.
- We added new components to the EMR Data Quality composite indicator.
