Learning Objectives Participants will be able to:

  • Appreciate the innerworkings of the FHT quarterly meetings and its influence/impact on regional change
  • Form subgroups that can focus on regional and sub-regional strategies including Health Links related initiatives
  • Leverage QIDSS on regional and sub regional initiatives.

Summary/Abstract Currently all 9 FHTs and 2 NP led clinics in the Erie St. Clair region meet quarterly to discuss organizational and regional strategies. Meetings kickstart with a QI update from update from the QIDSS and the latter half of the meeting involves the EDs and FHT/NP clinical leads breaking off to discuss relevant matters. The FHT/NP Clinial Leads share ideas and successful and unsuccessful implementations  and strategize. Topics include operationalizing: Patients First strategies, Health Links initiatives and best practices, Health Tapestry, fostering growth at an organizational level, QI strategies, strategies to work collaboratively with the LHIN with respect to specific initiatives and opportunities to further support one another. Takeaways are then shared with respective teams to champion change and process improvements. Quarterly meetings have helped FHTs establish an effective working relationship with the LHIN as well as other partners. Funding proposals and recommendations are discussed amongst the FHTs and NP Led Clinics and taken to LHIN leadership.  The FHTs and NP Led Clinics also provide input and direction to inform LHIN deliberations.
