- Increase their knowledge and understanding of health care consent and Advance Care Planning (ACP) in Ontario
- Review the role of Substitute Decision Making in advocating on behalf of patients
- Explore strategies used in Waterloo Wellington to assist primary care providers with introducing Advance Care Planning in “5 minutes or less “
- Review an EMR tool that has been developed to assist physicians in guiding their patients through the ACP process and documenting the details in the EMR
