A deep dive into co-designing care models for your priority populations

To help OHTs implement a population-health management (PHM) approach, RISE is hosting a series of webinars. This third webinar is based on feedback received from OHTs and builds upon the first webinar and second webinar . 

Specifically, this webinar will take a deep dive into co-designing care models and share additional examples for different priority populations. The learning objectives for this third webinar will help OHTs:

  1. redesign care models for their segmented priority population through co-design
  2. understand the key changes needed to improve outcomes for priority populations
  3. cultivate shared learnings by hearing from peer OHTs on their application of population-health management

This session will help support OHTs in achieving the following OHT TPA milestones:

  • Re-designing care for patients in your priority population(s)
  • Helping every patient in your priority population(s) experience coordinated transitions between providers

OHT Virtual Engagement Series - second session

    Measurement-Based Care in mental health and addictions service delivery

    Join the 2nd session of this series for senior administrative and clinical leaders from both hospitals and community organizations, and direct patient care providers.

    This session will highlight two examples of how organizations in Ontario have set up measurement-based care in practice. Presenters will discuss the implementation and change management considerations for organizations looking to establish measurement-based care practices in their settings. This webinar will be applicable to organizations from all healthcare settings

    Population-health management – An overview of core concepts, principles and RISE supports for OHTs

    To help OHTs implement a population-health management (PHM) approach, RISE is hosting a series of webinars.  The learning objectives of this first webinar are to help OHTs:

    • develop awareness of RISE’s population-health management supports for OHTs (including an introduction to PHM coaching)
    • understand the steps required to implement a population-health  management approach cultivate shared learnings by hearing from a peer OHT on their application of population-health management

    This session helps support OHTs in achieving the following OHT TPA milestones:

    • Re-designing care for patients in your priority population(s)
    • Helping every patient in your priority population(s) experience coordinated transitions between providers

    For cohort 1, this webinar will also provide a foundation for collaborative and coaching sessions which will follow (starting in January). For cohort 2, this webinar will provide an overview of population-health management approaches and visibility to upcoming supports.

    Digital Health/eServices for OHTs

    This webinar is targeted to all OHTs and will provide a brief update on the Digital First for Health strategy and an overview of the provincial eServices program. The Digital First for Health Strategy was announced in November 2019. The COVID-19 pandemic significantly accelerated the adoption of virtual care by providers and has expanded digital access for patients which provide greater access to health care services and secure digital access to COVID-19 test results. Digital approaches to health care have enabled the province to be better positioned to respond to the flu season and second wave of COVID-19 outbreaks.

    In April 2020, the provincial eServices program began development by bringing together eReferral and eConsult, two existing programs that have a demonstrated track record of success and would benefit clinicians by being integrated as part of the same workflow.  Other types of eServices are planned to be incorporated in the future.  The program is focused on integrating digital tools with the goal of reducing fragmentation in the current landscape and accelerating uptake and expansion across the province.  As part of this goal, the program will support local communities of practice including Ontario Health Teams (OHTs).  eServices can help OHTs support priority populations by enabling seamless and efficient electronic access to services and advice.

    By the end of the webinar, participants will gain an understanding of the digital supports available and be able to describe how they can leverage eConsult and eReferral to support care of their OHT priority patient populations.

     


    Supporting and engaging family caregivers in Ontario Health Teams

    The Ontario Caregiver Organization is pleased to offer a learning opportunity for Ontario Health Teams (OHTs). There are 3.3 million family caregivers in Ontario who play a critical role in integrated care, transitions, and health outcomes. The importance of caregivers has been recognized as a priority for Ontario Health Teams. This webinar will highlight resources and opportunities for OHTs to support and engage caregivers. Additionally, webinar participants will:

    • Learn about resources and programs offered by the Ontario Caregiver Organization that can help OHTs support and engage caregivers
    • Explore and discuss opportunities for the Ontario Caregiver Organization and Ontario Health Teams to work together to enhance supports for caregivers and engage caregivers in OHT development

    OHT Virtual Engagement Series

    This webinar is the first of the newly launched OHT Virtual Engagement Series, for enhanced and routine communications and engagement with teams at all stages of implementation. 

    Through this webinar, there will be discussions related to activities to support OHTs in their advancement during COVID response and updates related to ministry transformation priorities. Specifically, this first webinar in the series will provide teams with: key OHT implementation updates regarding Transfer Payment Agreement (TPA) deliverables, advancing provincial coverage, home and community care modernization, and an overview of upcoming OHT activities and milestones at-a-glance. There will also be an opportunity for teams to share priorities and feedback and ask questions of the ministry and support partners.
