Ministry of Health and Ontario Health is inviting you to an “OHT Virtual Engagement Series” information session to answer your questions about the recently updated data packages on Thursday, July 22, 2021 from 12:00 p.m. to 1:00 p.m., supported by the Ministry of Health and Ontario Health.
 
This one-hour Data Package and Data Supports Information Session will include a short overview of the ministry’s attribution methodology and new data package elements as outlined in the Ontario Health Teams: Data Supports Guidance Document you received with your data package, as well as an extended opportunity to ask any questions about the data packages. We strongly recommend that you review the Data Supports Guidance Document prior to attending the session.

About the OHT Data Package:
 
In early July, all approved Cohort 1 and 2 teams received an updated data package from the ministry. The data package provides a snapshot of the multispecialty physician network with which your team is best aligned and reflects how patients attributed to your OHT access care. It details your patient population’s health conditions, health system utilization, and associated health care expenses. New in the 2021 update, all data packages now include more recent data (from 2019/20), more detailed location information, socioeconomic analyses, and COVID-19 data. 
 
The ministry has also developed the Ontario Health Teams: Data Supports Guidance Document. The purpose of this document is to enhance teams’ data literacy by providing a detailed written walkthrough of the attribution methodology and each element of the data package, and by answering the most common questions received from teams. 
 

About the OHT Virtual Engagement Series
The OHT Virtual Engagement Series is part of a renewed approach to communications with teams. Working together with OH and partners in the Central Program of Supports, the ministry is looking to expand opportunities for teams to learn, ask questions, and share successes or challenges about areas of common interest. Through ongoing engagement and learning opportunities, teams will be better supported to achieve key milestones and advancement of the OHT model.
 
Future webinars will continue to highlight leading OHT initiatives across key elements and building blocks of the OHT model. Please contact your ministry point of contact if you would like your OHT’s activities to be highlighted through an “OHT Spotlight” in a future webinar.
