General Overview
The Sharbot Lake Family Health Team (SLFHT) is a busy rural practice located in the town of Sharbot Lake in the township of Central Frontenac. The SLFHT is a community based not-for-profit organization dedicated to providing quality, patient-centered health care for individuals and families in Sharbot Lake and surrounding communities. The Sharbot Lake Family Health Team includes two Family Physicians, Psychiatrist, Nurse Practitioner, Nurses, Dietitians, Occupational Therapist and a range of other health care professionals, Executive Director, Program Manager and administrative support staff who are committed to working together to provide comprehensive, accessible, coordinated primary health care services.
In 1992, the Ministry of Health and Long-Term Care (MOHLTC) announced diabetes reform as a strategic priority. In consultation with health providers, diabetes educators and people affected by diabetes, a strategic plan and vision statement were developed in support of diabetes reform in the province. The Ontario Diabetes Strategy (ODS) was announced in June 2008 and is a comprehensive strategy designed to expand diabetes programs and improve health and health care for Ontarians impacted by diabetes. In 2009, Sharbot Lake Family Health Team was chosen as a host site for the Diabetes Education Program. Their Diabetes Education Team includes Certified Diabetes Educators who provide care to over 500 residents of Central and North Frontenac region with diabetes or prediabetes.

The Diabetes Education Team includes a Registered Nurse CDE and Registered Dietitian CDE. Both are licensed health care professionals who specialize in helping people with diabetes and prediabetes learn how to manage their condition. 

Under the direction of the Executive Director, Sharbot Lake Family Health Team, the Registered Nurse CDE, will be a part of the inter-disciplinary team of Physicians, Nurse Practitioner, Psychiatrist, Nurses, Registered Dietitian CDE, Occupational Therapist, Executive Director, Program Manager and Administrative/Support staff at the SLFHT. 

Position Summary
In collaboration with the SLFHT you will assess, plan, and provide care coordination and evaluation to patients who are referred to for diabetes care and management. As part of the Diabetes Education Program (DEP), you will work with the DEP Dietitian to provide diabetes care to the patients of SLFHT, as well to community clients. 
A background in primary care, adult education, and diabetes prevention and management is required. Additionally, the RN will collaborate within the interdisciplinary team of health care professionals across the community network that influence the health environment targeted to the patient population, as well as within the SLFHT’s structure, in support of goals that align with the SLFHT’s strategy, Mission, Vision, and Values. The collaborative practice of this position includes accepting referrals from, consulting with, or referring to other health care providers in order to promote comprehensive and continuous care in both primary and acute care specialty practice. 

Key Duties and Responsibilities

  • Provide a thorough nursing assessment of patients with diabetes taking into consideration the patient’s level of knowledge and readiness, socio-economic and cultural background
  • Demonstrate understanding of stages of behavior change and ability to counsel individuals on these stages through development of behavior change goals
  • Triage referred patients with prediabetes or type 2 diabetes to determine health status, diabetes self-management skills and learning needs, diabetes management and glycemic control
  • Provide diabetes self-management education to people with diabetes and their families or significant others both individually and/or in groups
  • Use motivational interviewing techniques and adult learning principles to facilitate learning and support patient’s self-management goals
  • Use age-appropriate learning methods, behavioral/motivational strategies and reinforcement techniques
  • provide information to patients about the disease process in a matter tailored to their learning ability
  • Provide safety and health education to individuals and families
  • Develop patient centered health maintenance plans related to diabetes
  • Collaborate with patient and family members in setting short- and long-term goals; helping patients to take steps toward accomplishing goals
  • Works in collaboration with other members of the healthcare team in the best interest if the patient
  • Collaborate with patients and other health professionals to evaluate patient progress toward achieving management goals, both verbally and through written documentation
  • Ensure continuity of care by maintaining up to date medical records, communication with the patient’s primary care provider and other service providers as required
  • Provide regular monitoring of patients within scope of practice, along with other clinical providers
  • Demonstrate expertise in diabetes management in all aspects of patient care
  • Contribute to overall program planning and development
  • Using best practice guidelines, make decisions and recommendations for the introduction and titration of diabetes medications
  • Consider factors related to safety, effectiveness, evidence based-practice, and cost in planning and delivering patient care, and assists patient and family in identifying and securing appropriate and available services to address health-related needs
  • Practices ethical decision making by providing care in a nonjudgmental and non-discriminatory manner that is sensitive to patient diversity preserving patient autonomy, dignity and rights
  • Provide teaching to Family Medicine Residents, other students and inter professional team members

Education and Experience Requirements

  • Certificate of competence from College of Nurses of Ontario (RN status)
  • A member in good standing with the College of Nurses of Ontario
  • Baccalaureate Degree in Nursing (BScN)
  • Certified Diabetes Educator (CDE) with the Canadian Diabetes Educator Certification Board (preferred), or willing to become certified within two years of employment
  • Experience in Diabetes Education
  • minimum 2 years RN experience, preferably in a Family Health Team setting
  • Excellent knowledge of insulin and Oral Anti-Hyperglycemia agent (OHA) treatment based on the 2018 Clinical Practice Guidelines
  • Experience assessing and counseling diabetic patients
  • Experience working in primary or community care, preferably as part of an interdisciplinary team
  • Experience with principles of adult education
  • Experience with program development and the ability to support and provide input into the program evaluation process to align and determine priorities 
  • Foot care assessment and teaching an asset

  • Able to work independently with minimal supervision
  • Able to establish positive and supportive relationships with patients, families and Allied Health Care Professionals

  • Proficiency with various computer programs and application, especially Microsoft Office, TELUS (PS Suite)
  • Excellent attendance and employment record required
