The Niagara North Family Health Team is an integrated, collaborative, teaching family health team providing excellent care to patients in the Niagara region. Our vision is to work with our patients and families towards optimizing their health status and well-being. Our focus is on health care, health promotion, disease prevention and chronic disease management.
The Niagara North Family Health team is looking for a creative, innovative and highly motivated Program Administrator to work in our collaborative care model. This individual will provide technical, administrative and program support to all sites, programs and services within our family health team. Reporting directly to the Executive Director, the Program Manager is responsible for aiding in the oversight of all site EMR’s, quality improvement initiatives, data integrity and project implementation. Strong IT skills will be used to support our Quality Improvement Decision Support Specialists as well as our Quality Improvement Committee in order to optimize data management for statistical reporting as well as for the achievement of our quality improvement goals across our inter-disciplinary team.
Program management experience in a community based not for profit setting
Demonstrated track record in the application of technology to meet patient needs
Excellent verbal and written communication skills
Demonstrated initiative, critical thinking and problem solving skills
Strong interpersonal and facilitation skills
Experience in a health care setting an asset, preferably within a quality improvement role.
Proficient in the use of computer and information technologies, experience with electronic health records an asset including the development and implementation of forms and templates
Ability to work independently and cooperatively in a busy multidisciplinary team
Flexible in time management and able to move between sites as issues arise
