Job Description
As a member of the Health for All Community Care Team (CCT), the NP-PHC provides a range of independent and collaborative advanced nursing services with a focus on health promotion as well as the assessment and treatment of acute and chronic primary health conditions while working with family physicians and inter-professional healthcare providers. 

Duties & Responsibilities 

  • Perform individual assessment, diagnosis, treatment, health education/counseling and other clinical activities according to CNO standards of practice for RN (EC)
  • Provide care that focuses on prevention, chronic disease management, acute and episodic care
  • Team lead for the Community Care Team of interprofessional clinicians
  • Willingness and ability to provide safe home visits for patients facing barriers to accessing care in the clinic
  • Application and understanding of approaches to care that respect wholistic health and well-being, with an openness to the unique needs and beliefs of each individual
  • Provide coverage for other clinicians as needed
  • Participate in other activities as requested in collaboration with the CCT, the inter-professional team and local stakeholders including community presentations and outreach activities

Qualifications & Experience

  • Current registration with the College of Nurses of Ontario as an RN (EC) - Nurse Practitioner-Primary Health Care (PHCNP), required
  • Liability protection through the Canadian Nurses Protective Society (CNPS)
  • Membership in the Registered Nurses’ Association of Ontario & Nurse Practitioner Association of Ontario, preferred
  • Clinical leadership experience an asset
  • Experience as a Nurse Practitioner, with preference to those with experience in primary health care or in a community setting
  • Experience working effectively with communities and individuals who face barriers accessing health care and other supports, preferred
  • 3 years experience as a Nurse Practitioner in a primary health care setting or a combination of hospital, community and/or public health experience, preferred
  • Strong clinical skills, eexperience and proficiency in current physical assessment and treatment methods within the NP scope of practice in primary health care
  • Nursing experience working with priority populations and working within a multi-disciplinary setting
  • Competency in computer programs and experience with electronic medical records (Telus Practice Solutions preferred)
  • Excellent oral and written skills
  • Ability to work as a member of an inter and multi-disciplinary team
  • Ability to prioritize, manage time effectively and be flexible in an active work environment
