Job Description
The New Vision Family Health Team (NVFHT) is seeking a 1.0 FTE Primary Care Nurse Practitioner. This will be a contract position for 18 months to cover a maternity leave. The NVFHT is a multi-disciplinary team located in the Waterloo Region providing primary care to 28,000 patients. We offer a friendly, collaborative, and professional environment in a spacious facility.
Position Summary:
As an integral member of the NVFHT, the Primary Care Nurse Practitioner will provide comprehensive health care services to patients within full scope of practice with a strong focus on acute and episodic care and well-baby care. This exciting opportunity will appeal to a self-directed professional who has other skills and interests where they can build/expand upon our existing services.
Main Responsibilities:
- Diagnosis and treat patients for acute / episodic illness.
- Order appropriate investigations; interpret reports and analyze information to monitor progress and treatment plan.
- Provide care to unattached (unrostered) babies in community (up to 6 months of age).
- Conduct periodic examinations and risk assessment that may include care of at-risk populations, wellness exams, pediatric care, and immunizations.
- Recommend and arrange for referrals and appointments.
- Develop and maintain supportive, positive relationships with patients and their caregivers.
- Collaborate and consult with physicians and other interdisciplinary health providers.
Education, Experience and Skills:
- Current extended class certificate and unrestricted registration with the College of Nurses of Ontario (Masters or NP-PHC).
- Current membership with RNAO/NPAO, including professional liability insurance program.
- At least one year of clinical experience is preferred, particularly in primary care.
- Ability to work effectively both independently and as part of a team.
Job requirements:
- This position requires successful candidate to work onsite. We provide a dedicated office and examination room for these purposes.
- Willingness to work one evening per week is strongly preferred.
- A criminal background/vulnerable sector check.
- Be fully vaccinated against Covid-19.
