The Thames Valley Family Health Team is one of the largest Family Health Teams (FHT) in Ontario. With more than 115 physicians across 19 sites within London/Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive, collaborative primary health care to over 158,000 patients.

 

Family Health Team staff includes nurse practitioners, nurses, social workers, pharmacists, dietitians and other health care professionals who will work with physicians as an interdisciplinary team, each using their experience and skills to provide patient centered care.  For more information about our organization, please visit www.thamesvalleyfht.ca.

 

Role Summary:

Pharmacists are the medication management experts of the health care team. Through a collaborative, patient-centered model, the pharmacist will primarily be involved with providing pharmaceutical care.  The majority of time will be spent in direct patient care, identifying and resolving medication related concerns.  The pharmacist will also serve as a credible source of drug information to the team, and provide relevant patient / clinical education as appropriate.

 

Requirements:

·         Bachelor of Science in Pharmacy from a recognized university

·         Registration with the Ontario College of Pharmacists (Part A)

·         Certified Diabetes Educator (CDE) considered an asset

·         Two years’ experience preferred, or community/hospital residency and/or Pharm D an asset

·         Previous Primary Care experience is an asset

·         Experience with Academic Detailing is an asset

·         Demonstrated ability in problem solving using sound judgment and critical thinking

·         Experience in providing patient-centered care and evidence-based unbiased drug information

·         Experience in program development, implementation, monitoring and evaluation

·         Proof of professional liability insurance

 

Skills/Abilities:

·         Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction

·         Demonstrated leadership, organization, research, evaluation, time management and interpersonal skills

·         Experience and commitment to working collaboratively within a multidisciplinary team an asset

·         Demonstrated computer literacy is required to facilitate the use of electronic medical records

·         Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences

·         Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating a patient

·         Strong communication skills

·         Exhibit an ability to be open and non-judgmental

·         Knowledge of the goals and structure of the organization

·         Specific knowledge on Memory Clinic and Chronic Pain is considered an asset
