Job Description

Summerville Family Health Team is seeking a part-time Kinesiologist that will be comfortable working to their full scope of practice by helping patients improve their quality of life, sleeping habits, immune system function, and body composition. The Kinesiologist is an integral part of our interdisciplinary team supporting, developing, implementing and evaluating various programs that the FHT offers. 

Duties and Responsibilities 

  • Support the CHANGE program in collaboration with the health promotion specialist, the dietitians, and partners with Metabolic Syndrome Canada;
  • Assess patient health care needs (physical, emotional, psychological, and spiritual) to identify appropriate interventions;
  • Collect data through a therapeutic relationship with a patient;
  • Help patients to identify and use health resources, including those available in the community and from community agencies;
  • Develop and deliver health education programs for patients or communities; 
  • Develop and maintain collaborative partnerships with other organizations in the community to support SFHT programs and patient care; 
  • Maintains accurate, concise and confidential EMR documentation pertaining to patient care and ensures ongoing communication with the care team. 
  • Engages with community partners to plan and/or deliver services with the goal of improving the health of people in our community (e.g. MOHT initiatives).
     

Qualifications and Skills 

  • Bachelor’s Degree in Kinesiology required;
  • Registered member in good standing with the College of Kinesiologists of Ontario required, with evidence of current malpractice liability insurance;
  • Annual Cardio-pulmonary Resuscitation certification; 
  • Minimum 1-year experience with rehabilitation exercise and therapy would be an asset;
  • Thorough knowledge of kinesiology practice throughout the life span;
  • Knowledge and understanding of geriatric syndromes, age related changes and rehabilitative medicine;
  • Able to identify, select, develop, and prescribe intervention strategies to maintain, rehabilitate, or enhance movement and performance based on assessment findings;
  • Able to design customized exercise prescription for seniors with pathology, including but not limited to flexibility, strength, endurance, balance, and cardiopulmonary training, and corrective movement patterning;
  • Experience in the CHANGE program an asset;
  • Demonstrate understanding of therapeutic modalities and treatment applications.
