Job Description
The Healthcare Navigation First Nation Community Co-Design Lead will talk with people from the three local
First Nation communities and the local urban Indigenous community to understand how Indigenous Health
Service Providers and Indigenous Peoples find information about health care and social services today so
that we can make improvements to finding this information in the future. This individual will work together
with the Western OHT Health Care Navigation Lead and the Western OHT Operations Team to build a list of
organizations and people that help patients/clients and caregivers find healthcare services (e.g., Home and
Community Care Nurse) and a list of what directories are used to find healthcare information (e.g.,
thehealthline.ca). This individual will also seek advice from community members about how we should build
a local system to allow patients/clients, caregivers, and providers to find healthcare information 24/7.

QUALIFICATIONS:

  • Keen interest in improving the health care system
  • Excellent interpersonal skills; history of building and maintaining trusting relationships
  • ¬†Strong communication skills; ability to communicate effectively both orally and in writing
  • Self-starter; ability to organize time and work
  • ¬†Computer literate (e.g., Microsoft Outlook, Excel, Word)

EXPERIENCE:

  • High school diploma or equivalent preferred
  • Strong understanding of local First Nation communities
  • Experience in Indigenous health care

OTHER CONSIDERATIONS: Preference may be given to Indigenous candidates with relevant on reserve
employment and/or those with knowledge and understanding of Munsee-Delaware Nation or Oneida Nation
of the Thames or Chippewas of the Thames First Nation communities.
